If you are seeking information held by Corrective Services NSW (CSNSW) about a family member, you can lodge an access application under the Government Information (Public Access) Act 2009 (GIPA Act). An application form does not have to be used. The application can be made in the form of a letter. It must be stated in the letter that it is an access application under the GIPA Act.
If the family member is deceased, a copy of the death certificate will be required. The certificate will have to be sighted by a staff member of this Unit or certified by a Justice of the Peace.
Family member dead 30 years or less
If the family member has been dead 30 years or less, you will need to also provide documentation showing that you are the next-of-kin. Preferably you will be the closest relative. If not, consultation with the closest relative may be required. In addition, you will need to provide proof of identity. Usually, that is a copy of your driver licence. All the aforementioned documentation will have to be sighted by a staff member of this Unit or certified by a Justice of the Peace.

If you require information about a living family member, consultation with that family member must be attempted by this Unit before consideration is given to releasing the requested information.
